We have previously reported about a potential new neuromuscular blockade reversal agent called adamgammadex.
Its structure is quite similar to sugammadex, the core is gamma-cyclodextrin, however the side-chain was changed from mercaptopropionic acid to a branched one, thus the molecule became chiral. The chiral environment may increase the steric hindrance of the carboxyl groups, thereby strengthening its ability to bind rocuronium and reducing the risk of anaphylaxis induced by adamgammadex.
Phase III studies were concluded compared the efficacy and safety of adamgammadex and sugammadex. 310 subjects were randomized to receive adamgammadex (4 mg kg-1) or sugammadex (2 mg kg-1) at reappearance of the second twitch of the train-of-four (TOF), and standard safety data were collected.
Conclusion: Adamgammadex was non-inferior to sugammadex with a possible lower incidence of adverse drug reactions compared with sugammadex. Adamgammadex may have a potential advantage in terms of its overall risk-benefit profile.
